Education

What is nail fungus (onychomycosis)?

Nail fungus, medically known as onychomycosis, is a fungal infection that gets into the nail through small cracks in the nail or surrounding skin. It is caused by microscopic organisms called dermatophytes, the same family of fungi behind athlete's foot, along with less common yeasts and molds.

It usually starts small with a spot of white or yellow discoloration under the tip of the nail. Left untreated it tends to spread deeper into the nail causing thickening, crumbling edges and a change in shape. Toenails are affected far more often than fingernails largely because feet spend more time in warm damp environments where fungus thrives.

Risk factors

What causes nail fungus?

Nail fungus develops when fungi that normally live harmlessly on skin get the chance to invade a nail. This usually happens through a small injury, a crack from a manicure or pedicure tool or prolonged moisture exposure.

Risk is higher for people who:

  • Are older, since nails grow slower and thicken naturally with age
  • Have diabetes or poor circulation
  • Sweat heavily or wear closed toe shoes for long periods
  • Walk barefoot in communal showers, locker rooms or pools
  • Have a minor nail or skin injury
  • Have a weakened immune system
Warning signs

What are the symptoms of nail fungus?

Nail fungus usually develops gradually so early signs are easy to dismiss as a stain.

Common symptoms include:

  • Yellow, brown or white discoloration, usually starting at the nail tip
  • Thickened nail that is harder to trim
  • Brittle, crumbly or ragged edges
  • Nail pulling away from the nail bed
  • A slightly foul odor
  • Distorted nail shape in more advanced cases

Nail fungus is rarely painful in its early stages. Pain or swelling around the nail can be a sign the infection has progressed or that there is a secondary bacterial infection and is worth flagging to your provider.

Treatment options

What are the treatment options for nail fungus?

Your physician selects the right therapy based on severity, nail involvement, and your health history.

Most effective

Oral Antifungal Medication

Taken by mouth, oral antifungals treat the infection from the inside and are generally considered the most effective option for moderate to severe nail fungus, particularly toenails. Full nail regrowth typically takes several months since the medication works on new nail growth rather than the already infected nail.

Mild or early stage

Topical Antifungal Treatment

Applied directly to the nail, topical treatments are best suited to mild or early stage infections or for patients who are not good candidates for oral medication. They generally take longer to show results since they need to penetrate the nail itself.

Advanced cases

Combination Therapy

For more stubborn or advanced cases a physician may recommend pairing oral and topical treatment for a more complete approach.

Honest answers

Do home remedies for nail fungus actually work?

If you searched for nail fungus treatment before landing here you have probably seen tea tree oil, vinegar soaks or Vicks VapoRub mentioned as fixes. The honest answer is that evidence for these is limited and inconsistent. Some people report mild improvement with tea tree oil or vinegar soaks on very early superficial discoloration but there is no strong clinical evidence they clear a true fungal infection and they do not reach the deeper nail the way a prescription treatment does. Home remedies are also slow and most people trying them see the infection persist or worsen while waiting for results.

If you already tried an over the counter or home approach without success that is actually one of the more common reasons people come to us. It usually means the infection needs a prescription strength approach to clear.

Prevention

Is nail fungus contagious?

Yes. Nail fungus can spread to other nails, to the skin where it commonly shows up as athlete's foot, and less commonly to other people through shared surfaces like nail clippers, shower floors or nail salon tools. It is not highly contagious the way a cold is, but basic precautions such as not sharing nail tools and wearing sandals in communal showers reduce the risk of spreading it further or picking up a new infection.
